Step #1Sign in with one of our Patient CARE Specialist at either of our clinics. Step #2Paperwork! Although a "necessary evil", it's not that bad, in fact, one of our Patient CARE Specialists will help you in any way necessary to "ease the pain". It gives vital history information for the doctors to implement a personalized plan specific to your needs. As a new service for our new patients we now offer you the convenience of online forms. You may download the form in Acrobat® Reader® and print the document and fill it out before you come to our office. You can complete the form in the comfort of your home and avoid not having the information needed to complete the form. This will also allow you to have the majority of your paperwork done when you arrive. If you are not comfortable using the online form, come in and allow us to assist you with it. Be aware that the basic software file is 8.7MB and the full version (recommended) is 15.9MB. You will probably have the opportunity to use this again as there are more and more Web sites that require this software to view documents.
Step #3One of our staff members will escort you to one of our exam/consultation rooms to meet one of our doctors, and begin discussing the background history of your condition. Step #4You will then be given privacy to put on a gown. The doctor will return to give you a thorough chiropractic, orthopedic and neurological examination. We will take x-rays, if necessary, to be read by one of our doctors before your next visit. This generally will conclude your first visit with us. Our priority is to gather information on your first visit and present findings and recommendations on your second visit. Second VisitStep #5Before you arrive, the doctor has spent ample time reviewing your history, exam and x-ray findings. Now we will develop a personalized plan to meet your specific goals. We encourage your input and welcome your questions. Recommendations may include: Step #6One of our on-site Financial Specialists will meet with you to review your insurance benefits and discuss our financial policy.
